Client forms
To save yourself precious time on the day of your appointment, you can fill our important forms before you arrive. Click the link below to download the forms you need. Print them out and fill them in to the best of your ability, and a member of our staff can help you complete them when you arrive if needed.
Payment information
All payments for procedures must be made in full by the date of the procedure. Some procedures require a 50% booking fee ahead of time that needs to be paid in full 7 days prior to the procedure. We accept cash, checks and all major credit cards. If you need financing help for your procedures, we also are proud to accept the CareCredit healthcare credit card, North America's leading patient healthcare financing program. The CareCredit card is designed specifically for your health and beauty needs. Best of all, CareCredit offers special financing on purchases of $200 or more. Cancellation policy
We ask that all clients give us a notice of cancellation with at least 24-hours before their appointment. This helps ensure that other patients are not affected.
